Bipolar disorder is one of the most misunderstood psychiatric conditions. It's not just "mood swings" — it's distinct, treatable shifts between depressive episodes and elevated states (mania or hypomania) that can disrupt every part of life. At MJB Holistic & Therapeutic Services in Upper Marlboro, Maryland, we evaluate, diagnose, and treat bipolar disorder with the steady, evidence-based care this condition requires. We treat bipolar the way we treat every condition: by looking at the whole client, and not the hole in the client.

How bipolar disorder shows up

Bipolar exists on a spectrum. The most common patterns we see in our Maryland practice include:

  • Depressive episodes — persistent sadness, fatigue, loss of interest, sleep changes, hopelessness

  • Manic episodes — elevated energy, decreased need for sleep, racing thoughts, impulsive decisions, sometimes grandiose thinking

  • Hypomanic episodes — a milder version of mania that can feel productive but often precedes a crash

  • Mixed states — depression and elevated energy at the same time, which can be the most dangerous and disruptive

  • Cycling — moving between states over weeks, months, or sometimes within days

  • Periods of normal mood between episodes (which is why bipolar is often missed for years)

Bipolar I involves full manic episodes. Bipolar II involves hypomania (less severe) and depression. Cyclothymia is a milder, chronic version. We treat all subtypes.

Why accurate diagnosis matters

Bipolar disorder is often misdiagnosed as depression because patients usually seek help during depressive episodes. Standard antidepressants alone can sometimes destabilize bipolar — pushing a depressed patient into mania. This is why a thorough evaluation matters. Our approach to bipolar treatment

Bipolar disorder is highly treatable when matched with the right medication regimen and consistent monitoring. Treatment at MJB typically includes:

  • Mood stabilizers (lithium, valproate, lamotrigine, etc.) as the foundation

  • Antipsychotic medications when needed for mania, mixed states, or augmentation

  • Careful, slow medication adjustments — bipolar treatment is not about quick fixes

  • Pharmacogenetic testing for patients with complex medication histories

  • Coordination with therapists for psychotherapy (CBT-BD, family-focused therapy, IPSRT)

  • Sleep hygiene and circadian rhythm work — sleep is medication for bipolar

  • Regular follow-up to catch early signs of mood shifts before they escalate

Medications and pharmacogenetic testing

If medication is part of your plan, MJB Holistic offers in-house pharmacogenetic testing — a simple test that helps your provider tailor psychiatric medications to your specific body chemistry. Patients who use pharmacogenetic testing are roughly 2.5 times more likely to achieve remission, with fewer side effects and fewer trial-and-error medication switches. What to expect at your first visit

First appointments are 60 minutes. We'll talk through your full mood history — depressive episodes, any periods of unusual energy or productivity, sleep patterns, medication history, and family history. Bringing a partner or family member to the first appointment is welcome and often helpful — they sometimes notice patterns that are hard to see from the inside. Appointments can be in-person at our Upper Marlboro office or via secure telehealth.

Frequently asked questions about bipolar treatment

How is bipolar different from depression?

Depression is one direction — persistent low mood. Bipolar involves both directions: low mood (depression) AND elevated mood (mania or hypomania). Many people with bipolar are diagnosed with depression first because depressive episodes are what bring people to treatment. The presence of past hypomanic or manic episodes — even if they felt good at the time — is what distinguishes bipolar.

Will I need to be on medication for life?

Most patients with bipolar I do best on long-term mood stabilizer therapy. Going off medication is a leading cause of relapse. Bipolar II can sometimes be managed differently. Either way, we'll work with you on a plan that fits your life — not the other way around.

Can I drink alcohol on bipolar medication?

Most bipolar medications interact poorly with alcohol — both pharmacologically and because alcohol disrupts sleep, which is a major mood trigger. Most patients we work with reduce or eliminate alcohol; we'll talk through the specifics for your medication.

What about pregnancy and bipolar medication?

This is a careful, individualized conversation. Some bipolar medications are safer in pregnancy than others. Untreated bipolar in pregnancy carries its own risks. If you're pregnant or planning to be, we'll work closely with your OB to optimize the plan.

How quickly will I feel better?

Acute mania can be brought down within days to weeks with the right medication. Bipolar depression typically takes 4 to 8 weeks. Long-term mood stability often takes several months and ongoing medication adjustments. Patience pays off.
