Our founder, Melveena Johnson-Bolden, PMHNP-BC, is a board-certified psychiatric-mental health nurse practitioner with 28 years of nursing experience. She specializes in psychopharmacology married with psychotherapy — meaning treatment isn't just "here's a prescription, see you in three months." We combine medication management, evidence-based therapy approaches, and lifestyle work that supports whole-person healing.

Anxiety treatment at MJB typically includes:

  • Medication management when clinically appropriate — using the lowest effective dose with the fewest side effects

What to expect at your first appointment

First appointments are 60 minutes and can be in-person at our Upper Marlboro office or via secure telehealth on the Spruce platform. We'll talk through your current symptoms, your history, what's worked and what hasn't, and the kind of life you're trying to get back to. By the end, you'll leave with a starting plan — not a one-size-fits-all protocol.

You will not be rushed. Active leadership in your own care is part of how we work — we make recommendations and explain trade-offs, but you make decisions about your treatment.

Medications and pharmacogenetic testing

If medication is part of your plan, MJB Holistic offers in-house pharmacogenetic testing — a simple test that helps your provider tailor psychiatric medications to your specific body chemistry. Patients who use pharmacogenetic testing are roughly 2.5 times more likely to achieve remission, with fewer side effects and fewer trial-and-error medication switches. The test is often free for patients depending on insurance, with results in 3 to 5 business days.

In addition to in-person care at our Upper Marlboro office, we offer secure telehealth appointments through Spruce — a private, healthcare-focused platform built specifically for behavioral health. Telehealth is available across Maryland, with limited availability for patients in Washington DC and Georgia depending on Melveena's licensure in your state.

Frequently asked questions about anxiety treatment

How long does it take for anxiety medication to work?

Most anxiety medications take 4 to 6 weeks to reach their full effect, though many patients notice some improvement within the first 2 weeks. Short-term medications (used sparingly) can work within hours but are not first-line treatment for chronic anxiety.

Will I have to take medication forever?

Not necessarily. Many patients use medication for 6 to 18 months while building skills and stability, then taper off with provider support. Others stay on long-term because it gives them their best quality of life. The right answer is the one you and your provider arrive at together.

Can I get anxiety treatment without medication?

Yes. Some patients prefer to start with therapy alone, lifestyle changes, or non-medication interventions. We respect that and will support that approach if it's clinically reasonable.

Do you treat anxiety in adolescents?

Yes. Melveena treats anxiety in adolescents and adults of all ages. For very young children, we'll refer to a pediatric specialist when appropriate.

What's the difference between anxiety and a panic attack?

Anxiety is the ongoing worry, tension, and dread. Panic attacks are sudden, intense surges that often feel physically overwhelming — racing heart, shortness of breath, chest tightness, a feeling of impending doom. They can occur with or without a generalized anxiety diagnosis. We treat both.
