Trauma changes how the brain processes safety, memory, and emotion. The result — what we call PTSD — is not a character flaw or a sign of weakness. It's the nervous system doing exactly what it's wired to do after surviving something overwhelming. At MJB Holistic & Therapeutic Services in Upper Marlboro, Maryland, we treat PTSD the way we treat every condition: by looking at the whole client, and not the hole in the client.
How PTSD shows up
PTSD doesn't only happen to combat veterans, though we treat plenty of veterans and active-duty service members. PTSD can develop after any event your nervous system experienced as life-threatening or overwhelming: assault, a serious accident, medical trauma, childbirth, the sudden loss of a loved one, ongoing emotional or physical abuse, witnessing violence, or a long childhood of instability.
The symptoms we see most often in our Maryland practice include:
Flashbacks, nightmares, or vivid intrusive memories of the traumatic event
Hypervigilance — always scanning for danger, never fully relaxing
Easily startled, jumpy, or on edge
Avoiding people, places, conversations, or situations that remind you of the trauma
Emotional numbness, feeling cut off from people you love
Difficulty sleeping, including dreams that feel as exhausting as the original event
Anger that comes on faster and bigger than the situation calls for
Concentration difficulties or memory gaps
A persistent sense that the world is unsafe or that something bad is about to happen
Feelings of guilt, shame, or self-blame about what happened

Melveena Johnson-Bolden, PMHNP-BC, has worked with trauma survivors across her 28-year nursing career. PTSD treatment requires a careful, paced approach — pushing too fast can re-traumatize, and pushing too slowly leaves you stuck. We work at your pace.
PTSD treatment at MJB typically includes:
Thorough trauma-informed evaluation to understand what you've been through and what you need
Medication management when helpful — typically targeting sleep, hyperarousal, intrusive symptoms, or co-occurring depression and anxiety
Coordination with trauma-trained therapists when EMDR, CPT, or prolonged exposure therapy is part of your plan
Pharmacogenetic testing for patients who haven't responded to standard PTSD medications
Practical grounding tools you can use in real time when symptoms surge
Recognition that trauma recovery is non-linear and that progress takes time
Military and veteran care
MJB Holistic accepts Humana Military and TriCare. We work with active-duty service members, veterans, and military families. We understand the specific challenges of military trauma and the systems involved in service-connected mental health care.
Medications and pharmacogenetic testing
If medication is part of your plan, MJB Holistic offers in-house pharmacogenetic testing — a simple test that helps your provider tailor psychiatric medications to your specific body chemistry. Patients who use pharmacogenetic testing are roughly 2.5 times more likely to achieve remission, with fewer side effects and fewer trial-and-error medication switches. What to expect at your first appointment
First appointments are 60 minutes. You will not be asked to relive the worst moments of your life on day one. We'll talk about how you're functioning now, what symptoms are most disruptive, what's helped or hurt in the past, and what your goals are. Trauma details come up only when you're ready — we follow your lead.
Appointments can be in-person at our Upper Marlboro office or via secure telehealth on Spruce, whichever feels safer for you.
Frequently asked questions about PTSD treatment
Can PTSD be treated without me having to talk about the trauma in detail?
Yes. While some therapy approaches involve processing the trauma in detail (with proper preparation and pacing), much of PTSD recovery happens through skill-building, regulation, and treating the symptoms directly. Medication can also reduce nightmares, hyperarousal, and reactivity without you ever needing to recount what happened.
I had a traumatic experience but I'm not sure if it counts as PTSD. Is it still worth coming in?
Yes. You don't need a confirmed PTSD diagnosis to get help. Many people experience trauma symptoms that don't meet full PTSD criteria but still significantly affect their lives. Treatment helps regardless of the label.
What medications are used for PTSD?
First-line medications include certain SSRIs (sertraline and paroxetine are FDA-approved specifically for PTSD). Prazosin is often used for trauma-related nightmares. We may also treat co-occurring depression, anxiety, or sleep issues with appropriate medications. We never use benzodiazepines as first-line PTSD treatment per current clinical guidelines.
How long does PTSD treatment take?
It varies. Some patients see meaningful relief within 8 to 12 weeks. Complex trauma (especially from childhood or sustained over years) typically takes longer. Treatment often continues in some form for a year or more, but you should feel meaningful improvement well before then.
Do you offer EMDR or other specialized trauma therapy?
Melveena specializes in psychiatric medication management. For specialized trauma therapy modalities like EMDR, CPT, or prolonged exposure, we coordinate with trauma-trained therapists in the community. Many patients work with both us (for medication) and a therapist (for therapy) at the same time.

Telehealth is available across Maryland, with limited availability for patients in Washington DC and Georgia depending on Melveena's licensure in your state.
